If it is difficult for a guardian to accompany a minor during their stay at our facility, it is necessary for the guardian to sign a "Guardian Consent Form". Please have the guardian sign the consent form by hand and bring it with you on the day of your stay.

Please note that you will not be able to stay if there are any omissions or missing stamps on the consent form. Be sure to bring the original form and use a ballpoint pen or a permanent marker for the signature. Signatures made with pencils or erasable pens are invalid.
